Not interested in quitting yet, wants to work on smoking cessation first. Doesn’t drive. Discussed substance abuse treatment. Patient is interested in mental health clinic referral 3. Smoker-2 packs per day, wants to quit. Will recommend nicotine patches 4. Anxiety/depression-is on Paxil, also takes Xanax. Told patient will not prescribe Xanax with ethanol use. Will consult mental health clinic 5. Hypothryroidism-will check TSH levels today 6. GERD-lifestyle modifications for now 7. Sinusitis, chronic-do not carry Zyrtec, will prescribe Claritin 8. Low back pain-patient will continue Vioxx from outside provider 9. Mammogram within last 6 months, on calcium 10. Skin lesions-refer to dermatology 11. Return to clinic in 3 months
